Cytomegalovirus Role in Inducing Rejection Reactions in Heart Transplantation Patients

چکیده
The study was performed on 45 patients with heart transplantation out of which 30 presented serologic cytomegalovirus infection. For histological study we used: hematoxylin eosin, van Gieson, Masson's trichrome, green methyl pironin stains. Immunohistochemistry was performed using ABC technique (avidin biotin complexes or avidin biotin) with the following monoclonal antibodies: CD8, CD4, CD20, CD68, CD31, CD45 RO, HLA-DR, and E13. In many of our cases cytomegalovirus infection was commonly associated with acute rejection. The presence of endothelial cells expressing HLA-DR antigens and subendothelial accumulation of activated T lymphocytes and macrophages, often in contact with endothelial cells, highlights the role of cellular immunity in the production of accelerated atherosclerosis lesions.

Thymoglobulin is a purified polyclonal immunoglobulin that has been used widely over the last decades in the prevention and treatment of rejection following renal transplantation. This immunoglobulin works against human thymocytes. Since thymoglobulin does not contain the nephrotoxic properties therefore it can be used in induction therapy especially in patients with higher risk of graft reject...
متن کاملIncidence and risk factors for cytomegalovirus in kidney transplant patients in Babol, northern Iran
Background: Cytomegalovirus (CMV) disease is an important cause of death and possibly transplant rejection in kidney transplant (KT) patients. This study was conducted to investigate the incidence and risk factors of CMV disease in kidney transplant patients. Methods: All end-stage renal disease (ESRD) patients who underwent kidney transplantation during 1998-2014 and their donors were assessed...
متن کاملRole of Cytomegalovirus Reinfection in Acute Rejection and CMV Disease After Renal Transplantation
Renal transplantation is a most valuable treatment for patients with end-stage renal disease as it offers improved survival and quality-of-life benefits compared with dialysis (Evans, Manninen et al. 1985; Port, Wolfe et al. 1993). However, there has been no satisfactory increase in long-term graft survival despite significant advances in the field of renal transplantation (Meier-Kriesche, Scho...
متن کاملCytomegalovirus serology and replication remain associated with solid organ graft rejection and graft loss in the era of prophylactic treatment.
BACKGROUND Cytomegalovirus (CMV) replication has been associated with more risk for solid organ graft rejection. We wondered whether this association still holds when patients at risk receive prophylactic treatment for CMV. METHODS We correlated CMV infection, biopsy-proven graft rejection, and graft loss in 1,414 patients receiving heart (n=97), kidney (n=917), liver (n=237), or lung (n=163)...
متن کاملMechanisms of chronic cardiac allograft rejection.
Chronic rejection in the form of cardiac allograft vasculopathy is one of the major factors that affects long-term graft and patient survival after heart transplantation. Whereas multiple factors contribute to the development of cardiac allograft vasculopathy, immunologic mechanisms play the predominant role in the chronic rejection process, because both alloimmune and autoimmune responses are ...
